CBE, LTD. PREVENTATIVE MAINTENANCE SERVICE AGREEMENT
CBE, Ltd is pleased to offer a preventative maintenance (PM) program to for
a minimum of three (3) visits over the next calendar year beginning by a CBE, Ltd.
qualified field service technician.
The service agreement must be signed a minimum of ten (10) working days
prior to the first requested service date to be eligible. The months of all three
(3) service calls must be determined at the time the agreement is signed. A CBE,
Ltd representative will be in contact with plant personnel one month prior to the
next service call to schedule specific dates to complete the service. Maintenance
for this account will be scheduled for the months of ______________________ , ______________________
, & ______________________.
CBE, Ltd. PM Labor rates are as follows:
- $56.00 per hour for travel time on normal working days
- $75.00 per hour for labor with PM agreement (regular
service rate is $85 / Hour)
- $112.00 per hour for overtime (more than 8 hours of
plant time) and weekends
Terms:A purchase order to cover all three (3) service calls must accompany the signed service agreement. Work is based on a normal eight (8) hour day, with a minimum charge of four (4) hours. Overtime rates will apply to any time worked over eight (8) hours (not including travel time) on a given day. Payment terms are net 30 days from date of service. Failure to pay bills on time could result in revocation of preferred PM labor rate or cancellation of this service agreement. CBE, Ltd. will hold all labor costs at the above stated rate for a period of twelve (12) months from the date of customer signing of this agreement. After the first initial twelve (12) months of this agreement, CBE, Ltd. reserves the right to adjust its service rates or policies.
Travel / Living Expenses: All traveling and living expenses incurred by CBE, Ltd.’s technical representative on the customer’s behalf will be paid by the customer. Travel expenses include: transportation to and from airports, trains, or bus depots; air, train or bus fare; car rental charges; private car use by technical representative will be rated at $0.505 per mile. Living expenses include, but are not limited to, lodging and meals. Meals are charged on a per diem basis of $45.00 without receipts. When costs are in excess of this rate receipts will be submitted.
Parts: Under the CBE, Ltd. PM Labor Service Agreement, customers will be
eligible for a 10% discount off all parts regularly stocked by CBE, Ltd at our Taylor,
TX facility while the agreement is in force. No discount will be given for non-stocked
or special order parts (regular CBE, Ltd. pricing will apply)
CBE, Ltd. will provide proof of Liability insurance and proof of Workman’s Comp.
Insurance if required. If you require your company as additional insured on our
insurance policy, any additional charges for that service will be billed at additional
cost.
If you require our technician to attend a short on-site safety course before service
is performed, this is acceptable. However, you will be charged for the time spent
for this safety training.
